One time enrollment fee of $25 applies. Membership fees are due at enrollment and are valid for one year from the time of enrollment. Membership fees are nonrefundable and nontransferable. Benefits expire on an annual basis and do not roll-over. Membership plan is a savings plan, NOT a dental insurance plan. Membership plans cannot be combined with any other insurance plans or third party financing. It is ONLY valid at Ivory Dental Studio.
*To receive the $1000 credit for orthodontic treatment, the member must be enrolled in the membership plan for the entire duration of the treatment. Cannot be combined with other offers.*
